ESWL (Extra Corporeal Shock Wave Lithotripsy)


  • Extra Corporeal Shock Wave Lithotripsy (ESWL) or Simply Lithotripsy
  • It is a device for fragmentation of stone; and is used in management of renal , ureteric and bladder stones.
  • mechanism of action :  It generates shock waves, which are reflected ( with the help of reflector) to the stone .These shock waves cause the fragmentation of the stones.
  • Imaging of stones: It is done by high frequency 9" C - Arm Image Intensifier.
  • Indications for ESWL (Lthotripsy) :
  • Renal stone (calculi) : < 2.5 cm in size
  • Ureteric stones (calculi) : < 1.5 cm in size ( only for upper ureteric stones)

        (for middle/ lower ureteric calculi, URS ( Ureterorenoscopy) is the preferred mode of treatment)

  • Bladder stones (calculi): ESWL can be done; but the preferred mode of treatment is Cystoscopic Lithotripsy due to its cost effectiveness.

  • Preparation for ESWL:
  • Correction of UTI (Urinary Tract Infection), if any by, antibiotics, This is confirmed by urine culture and sensitivity.
  • Blood Investigations such as CBC (with platelets), Sr. Creatinine,
  • USG / X - Ray / IVP to determine tha size of the stones, and the extent of obstruction, if any.
  • To come nil orally for about 2 - 3 hours prior to the procedure.
  • Post ESWL Precautions :
  • To drink  plenty of water / liquids to flush out the fragments.
  • To continue antibiotics / painkillers if advised.
  • To report in case of fever / haematurie / persistent pain
